New Patient Information
First Visit
On your First Visit please arrive 15 minutes prior to your scheduled appointment time to fill out the necessary paperwork and bring any insurance or referral information that is necessary. If you have any MRIs, X-rays, or any diagnostic testing please bring the actual films if possible but the report can help until we acquire the images.
Also, please wear or bring comfortable clothes (T-shirt, shorts, jogging pants) so you feel comfortable and unrestricted during the examination process.
On your first visit we will complete a comprehensive history, examination and treatment of the pain, injury or ailment in concern. This initial exam will take 30 minutes to 1 hour, so please allow adequate time.
